Individual Support Planning
01) Get to know you.
In our kick-off meeting, we’ll chat with you and the people you invite to join in the process. We’ll cover topics like goals, hobbies, and any care considerations or risks.
Program Development
02) Create a plan.
Our experts develop a customized plan with optimistic, achievable benchmarks. We have experience writing plans that exceed state quality requirements.
Follow-up Meetings
03) Work toward goals.
Now it’s time to make your vision a reality! After identifying your next steps, we’ll meet consistently to review progress and make adjustments to your plan.

I joined Apace in 2015, first as a Community Support Coordinator in Bellevue for 6 years and now as the Community Outreach Director. Prior to Apace, I was Developmental Disabilities Service Coordinator and Children and Family Services Specialist for the Department of Health and Human Services. I have spent over a decade advocating for children, their families, and people with Developmental Disabilities in Nebraska.
